If you participated in reimbursement plans in 2017, the deadline to submit eligible claims to Mayo Clinic Health Solutions is just days away!  Submit claims by March 31st for 2017 expenses in these plans:

  • Dependent Care Flexible Spending Account
  • Health Care Flexible Spending Account
  • Mayo Reimbursement Account

If you're not sure about your balance, contact Mayo Clinic Health Solutions at 1-800-635-6671.  For your convenience, you can submit claims and documentation in one of three ways:

  1. Online via the Mayo Clinic Health Solutions website.
  2. Using the Mayo Clinic Health Solutions — Reimbursement mobile app (available through the iTunes and Google Play app stores)
  3. Using the paper claim form, which you can submit by mail, fax or email:

Claims from 2017 expenses received by Mayo Clinic Health Solutions after March 31 will be denied.
