Minnesota COVID-19 Child Care Verification Form

MN Executive Order

In response to Minnesota Executive Order 20-02, local schools and charters are required to provide care for elementary-age children of health care professional, first responders, and other emergency workers during previously planned school days to ensure Minnesota’s first line of defense against COVID-19 can stay on the job. As part of the school or charter’s effort to provide care to those where Executive Order 20-02 applies, they may request verification of an individual’s employment as a health care professional.

This letter (linked below) may be used to fulfill this verification if you have children who are enrolled in school, kindergarten through age 12, are not working remotely, and meet at least one of the following criteria:

  1. Your role is to provide direct patient care (e.g. nurse, respiratory therapist, doctor)
  2. Your role supports those who care for our patients (e.g. pharmacy technician, laboratory personnel, technical support for our patient care systems, hiring staff, etc.)
  3. Your routine work has been redirected to activities related to COVID-19. (e.g. administrative personnel being redirected to screen visitors coming onto the Mayo campus)

If you do not meet the above criteria, there are other ways in which Mayo Clinic is supporting you and your child care needs during this time of uncertainty. Please see this list of options for back-up child care in a community crisis.
