I wanted to inquire as far as there is not a description as to what qualifies as spouse for medical insurance purposes. Previous hospital employment required both names on a lease for 1 year as well as identification with the same address listed. Is a marriage certificate required for coverage?

Meg, the plans define spouse as the person to whom you are married lawfully. We do not require that you provide a copy of your marriage license, but it is expected that you have one.
