Dress code policy for OR employees

Hi, starting at Arizona location soon, and wanted to know if there is a certain dress code for coming into work before dressing out in facility provided scrubs? Additionally, what is the policy for head coverings specific to this location/OR. And lastly, how about nail polish? Thanks in advance!
