What happens if I decide to not finish my onboarding

Hi everyone, I have an interesting issue. My current employer is offering me a promotion to try to keep me. If I decide to not finish my onboarding would I be black listed from ever applying again at Mayo? I haven’t done my physical or HR appointment yet. Only the background check is started.
