Looking for a lo-tech way to claim your scholarship award?
Mayo Clinic recently announced that the Mayo Clinic Dependent Scholarship plan will not be able to make new awards this year. However, if you have a dependent who received a scholarship award in a prior year, they can continue to submit payment requests up to the plan maximum.
If you have access to the Mayo Clinic intranet, the online Scholarship tool is the most efficient method to submit that request. But, for those employees who are on furlough or who are working from home without access to the Mayo Clinic Intranet, a paper form is now available. Follow these simple steps:
- Click on the PDF file at the bottom of this post
- Print and complete the form.
- Mail the form to HR at the address provided.
