Wisconsin COVID-19 Child Care Verification Letter

In response to Wisconsin Emergency Order 12, Mayo Clinic wants to support the efforts of the Wisconsin Safer At Home order for non-essential business in Wisconsin. This order allows Mayo Clinic and Mayo Clinic Health System staff members with essential roles in health care operations to continue to come to work. The letter linked below, in combination with a Mayo Clinic Access Identification Card is verification that the individual is an active employee, not working remotely and meets at least one of the following criteria:

 

  1. Their role is to provide direct patient care (e.g. nurse, respiratory therapist, doctor)
  2. Their role supports those who care for our patients (e.g. pharmacy technician, technical support for our patient care systems, hiring staff, laboratory personnel)
  3. Their routine work has been redirected to activities related to COVID-19. (e.g. administrative personnel being redirected to screen visitors coming onto the Mayo campus)
