When an employee sustains an injury or becomes ill, they may be unable to perform certain job duties or need time off from work.  The staff of the Work Connect Program will guide and assist Mayo Clinic Rochester employees and their supervisor through the leave process and eventual return to work.  The Work Connect Program is an integrated disability management program comprised of staff from the Claims, Return to Work, and Integrated Disability Coordinator sections in Recovery and Claims Services (RCS), the Nurse Case Managers in Occupational Health Services (OHS), and Human Resources.  The Work Connect Program is applicable to employees who sustain work related injury or illness and non-work related injuries and illness, including maternity and paternity leave requests.

General Information

Mayo Clinic employees may qualify for job protection under the Federal Family and Medical Leave Act (FMLA) and wage replacement under either Workers' Compensation benefits (work-related injury/illness) or the Short Term Disability (STD) benefit (non-work related injury/illness). If an employee is not eligible for or does not qualify for job protection under FMLA, they may be eligible for a personal leave for a serious health condition or disability.  Review the information below to begin the process of submitting a leave and applying for benefits.

WORK-RELATED INJURY/ILLNESS

  1. Submitting an Employee Incident Report.
    • While is it preferred that the employee completes the Employee Incident Report, they may not be able to do so.  If the employee is unable to submit the Employee Incident Report, the supervisor should complete it on behalf of their employee.
    • NOTE:  If the incident resulted in the employee's hospitalization or death, contact the your local Safety/Occupational Health representatives immediately. 
    • Supervisors should review Work Connect Program information while on site at Mayo for further information.
    • If both the employee and supervisor are unable to submit an Employee Incident Report, another member of the department with either first-hand knowledge of the incident should complete the Employee Incident Report to begin the Workers' Compensation benefit and potential leave process.
    • An RCS Claims Representative will be assigned to the Workers' Compensation claim to review for approval/denial, handle payment of all reasonable and necessary medical services for approved claims, and process wages if the employee misses work for approved claims following the benefit waiting period.
    • The RCS Claims staff will also review the information to determine if the employee will be missing time from work and whether the employee is eligible and the leave qualifies under FMLA.  There is no need to submit a separate leave of absence request.

NON-WORK RELATED INJURY/ILLNESS

  1. Submit an FMLA leave.
    • While is it preferred that the employee submit their leave of absence request, they may not be able to do so.  If the employee is unable to request the leave of absence, the supervisor should complete the leave request on behalf of their employee. Supervisors should review the Work Connect Program information while on site at Mayo for further information.
    • Only an employee's direct supervisor can submit a leave on behalf of their employee if the employee does not have the ability to submit their own leave.
    • If an employee's direct supervisor is unable to submit a leave for an absent employee, the employee should contact RCS at 507-422-0505 and request assistance with submitting a leave.
