POPA Question: Vaccination Question

Hi,

I have quick question about the POPA appointment I saw that needed to bring my vaccination records. My doctors network allows me to print it off of their online medical system (MyChart) , would this be acceptable or do I need official ones from the doctor's office?

Thank you for your question and welcome to Mayo Clinic! For a POPA question, we would ask you to call your Occupational Health office directly. You should receive a telephone number (along with the online scheduling link) for Occupational Health in your Onboard portal with instructions. They will be able to assist you with the records you need. Thank you!!

I’m sure that would be ok. But just call office to make sure!

I did just that, I pulled out my phone and logged into mychart. The RN just copied my records from my phone

How do you get a copy of your vaccinations that are not recent. I submitted information on myir mobile
